Become a Patient

To get started on your journey back to health, please fill out the new patient registration form

You will fill in your information and then be redirected to register in our EHR (your patient portal). 

Within a few days, after you register, our New Patient Coordinator will contact you by Secure Message in the patient portal with instructions on how to schedule your first appointment. They will be available to discuss more about how we work with patients, the Assessment process, and our membership pricing plans with you.

The way that our clinicians work with patients differs considerably from the conventional medical model to which you may be accustomed. You may read more about our functional medicine approach here. 

Phase One: Assessment

The Assessment is designed to save money and time in the long-term by performing the appropriate diagnostic testing and evaluation before treatment begins. A cornerstone of our approach is “test, don’t guess.” Identifying the underlying patterns contributing to disease is the key to a successful and lasting outcome.

The Assessment process comprises two appointments, the Initial Consult and the Case Review.

The Initial Consult

At this initial 90-minute video appointment, we will take a comprehensive health history and order initial testing. The purpose of this appointment is for us to make sure that we have all of the information needed to gain a comprehensive understanding of many of the factors affecting your health. 

After the Initial Consult, your clinician will:

  • Recommend initial laboratory testing, specifically tailored to your personal case.
  • Provide you with dietary guidelines, which in some cases will include a 30-day Paleo reset diet.
  • Recommend initial supplements and lifestyle changes for you to begin.

Once you complete your tests and we have received the results, you will move on to step two: the Case Review. This typically occurs between 45 and 60 days after the Initial Consult, because some of the labs we use can take up to 4 weeks to deliver the results to us after receiving your sample. While you wait for your test results to come back, you will be implementing the dietary, initial supplement and lifestyle recommendations, and making forward progress.

The Case Review

Your second appointment is a 60-minute Case Review appointment where your clinician will

  • Integrate data from your history and intake with the new laboratory data from testing.
  • Discuss the underlying patterns and triggers that are contributing to your symptoms.
  • Outline the suggested treatment plan, including dietary, supplement and lifestyle recommendations.
  • Recommend further testing (if necessary).
  • Answer any questions you have about the findings or the treatment plan.

Phase Two: Follow-up Care

Once the Case Review is complete, you will have follow-up appointments on an as-needed basis. In general, it is recommended that regular follow-up appointments be scheduled every 6-8 weeks, depending on your health needs. At these appointments, your clinician will review your progress with the treatment plan, along with any follow-up test results, and make additional recommendations as necessary.

You may schedule follow-up appointments in the office, by phone, or by video. While you can become our patient as long as you reside within the United States, we do ask that you return for an in-person appointment at least once per year if you live outside a state in which your clinician is licensed, which are:
